General medical exam requirements and eligibility

For an administrative physical, your MinuteClinic® provider will conduct a detailed review of your medical history as well as your family's, any medications you take, allergies and current health status. They can review your immunization records to help ensure they are up-to-date.

  • After the evaluation, your MinuteClinic provider will complete any necessary medical forms.
  • Blood tests can be a part of the general medical exam service. Your MinuteClinic provider can administer required extra testing for an additional fee.

Parents are reminded to fill out the parents' sections of all the necessary forms before a child's or adolescent's visit. Doing so will help to simplify and speed up the physical exam process for you. Please collect the following medical information to bring to the examination:

  • All the documentation that your organization requires
  • Immunization history and records
  • Primary care provider's name and contact information
